A:  We are open at our new location at 11015 Olson Dr. Suite 6, Rancho Cordova CA 95670.   
Q: Will the practice be under new ownership?
A: YES, the office will go through a name change to Healthy Eyes Optometry Rancho Cordova, with Dr. Jing Han.
Q: Will Dr. Miyamura continue seeing patients?
A: She will see patients 2 days a week at the new office location.  Please call 916-635-1823 to schedule an appointment.
Q: Do I need to request a copy of my records or glasses or contact lens prescriptions?
A: No.  If you continue to have your  eye exams at the new location, you will not have to request copy of records.
Q:  What if I want a copy of my records anyway? 
A:  You can call or text the office at 916-635-1823, or click on Patient Forms, and email that request to [email protected].  HIPAA requires specific information, details and wording so please use the form provided to avoid delays.  Please note: Large requests or mailed records may involve a fee.  See Patient Forms for details.  
Please understand that we have a large number of requests and are doing our best to get them to you.  If you have an exam appointment scheduled, have that office fax us your signed request for records with examination date and we will expedite it directly to the office.
